All forms are due back to Stuart by Tuesday May 6. Please return forms to the attention of Christine Doidge.

You can now fill out the forms online, then print the filled-out forms. Please note that you must fill out the forms in one session as you will not be able to save the filled-out forms in digital format. If a form field is not wide enough for the information you wish to enter, you will have to fill in that field by hand.

Physical examinations are required for all new students and for those students entering Pre-School, Kindergarten, Grade 3, Grade 6, Grade 9 and Grade 11. The physician's report must be submitted prior to the start of classes.

In addition, a physical examination is required every year for students who wish to participate in Upper School sports. The physical examination must be completed and the physician's report submitted to the school beefore the student may participate in pre-season practices. No physical examination (except as listed above) is required for participation in Middle School sports.

The Physical Examination form is included in the registration packet this year.
